What’s included in the membership?
Two Routine Dental Cleanings: Regular cleanings are vital for maintaining oral health. Our thorough cleanings ensure your teeth and gums stay healthy.
Two Periodic Examinations: Receive personalized attention and expert advice during two exams each year to keep your oral health on track.
Two Fluoride Treatments: Help prevent cavities and strengthen your teeth with two fluoride treatments per year (for children under 12).
All Necessary X-rays: Stay ahead of any potential issues with comprehensive X-rays, including a full set every 5 years.
One Emergency Exam: In case of a dental emergency, one emergency exam per year is included to provide immediate attention and care.

What are the exclusions and limitations?
Discounts Do Not Apply to:
- SureSmile Clear Aligners
- In-office products (Clin-pro 5000, sleep products, Coco Floss, etc.)
- Cosmetic Procedures
What are the terms and conditions of the membership?
- Membership is for one year from the enrollment date and is non-refundable and non-transferable.
- No discounts on completed treatments or treatments in progress prior to purchasing membership.
- Services must be paid in full at the time of service.
- Treatment not completed within 90 days of diagnosis forfeits any discounts.
- Minimum 48-hour cancellation notice required for appointments to avoid charges.
- No discounts on services performed by referred specialists.
- Plan benefits cannot be combined with other promotions or carried over to subsequent years.
Are there any other important details I should know?
- If a hygiene appointment is failed, one of the covered no-charge cleanings and exams is forfeited.
- The membership must be paid in full before any discounted services contained within the membership package are covered.
- Services included in the annual fee can only be used in the plan year in effect.
What happens in the event of a dispute?
- Alex Kalmanovich DDS / Pearline Health shall be the sole party to determine participation eligibility, discounts, and eligible services rendered and covered under this Plan.
- The annual participation fee and discount percentages may change at our sole discretion.
- In the event of any dispute with respect to this Plan, a refund of the annual fee may be reimbursed to the member minus the value of all services performed to date and shall be the sole and final remedy at the option of Alex Kalmanovich DDS / Pearline Health.
